Coordinated spam affects multiple communities. Should detection rely on automation, staff observation, or both? How do you as an admin prevent false positives while keeping your forums safe?

I do a few things to stop spam, and so far, I've been catching 100% of it.

1. I do a security question related to the niche of the forum or the forum itself in some way or another.
2. I manually approve all new members.
3. I have a required question asking them why they want to join on registration.
